The Diabetes-Dementia Connection: A Growing Body of Evidence
The relationship between diabetes and dementia is becoming increasingly clear, with new research revealing how blood sugar problems impact brain health and vice versa.
According to recent studies, individuals with diabetes are approximately 60% more likely to develop dementia than those without the condition. Moreover, frequent episodes of low blood sugar have been linked to a 50% higher chance of cognitive decline.
One of the key factors contributing to this connection is insulin resistance, a major cause of type 2 diabetes. When cells stop responding properly to insulin, too much glucose remains in the blood, leading to complications. Insulin resistance typically affects the liver and muscles but also impacts the brain, particularly in Alzheimer’s disease. In this condition, brain cells may struggle to use glucose for energy due to insulin resistance, exacerbating cognitive decline.
This combination of poor glucose utilization and insulin resistance has been unofficially referred to as ‘type 3 diabetes.’ People with Alzheimer’s often exhibit higher fasting blood glucose levels, even if they don’t have diabetes, indicating a pre-diabetic state. Animal studies have also demonstrated that Alzheimer’s-like changes in the brain raise blood glucose levels.
Insulin Sensitivity and Alzheimer’s Risk
The highest genetic risk factor for Alzheimer’s, the APOE4 variant, reduces insulin sensitivity by trapping the insulin receptor inside the cell, where it cannot be activated properly. This highlights the complex interplay between insulin sensitivity and Alzheimer’s risk.
Furthermore, research has shown that blood vessel damage, a common complication of diabetes, can also impact the brain. High or fluctuating blood glucose levels can injure blood vessels in the brain, reducing blood flow and oxygen delivery. Diabetes can also weaken the brain’s protective barrier, allowing harmful substances to enter the brain and trigger inflammation. Reduced blood flow and brain inflammation are strongly linked to dementia.
Diabetes Research: A Potential Source of Dementia Treatments
Interestingly, some diabetes medications have shown promise in reducing dementia risk. Memantine, a medication used to treat moderate to severe Alzheimer’s symptoms, was originally developed as a diabetes medication. Although it didn’t succeed in controlling blood glucose, researchers later discovered its benefits for brain function. This highlights the potential for diabetes research to inform the development of dementia treatments.
Metformin, the most widely used diabetes medication, has also been found to have anti-inflammatory effects on the brain. Some studies suggest that individuals with diabetes who take metformin may be less likely to develop dementia, and those who stop taking it may see their risk increase again. Trials are currently underway to test the effects of metformin in individuals without diabetes.
GLP-1 receptor agonist drugs, such as semaglutide (Ozempic, Wegovy), have also been found to reduce dementia risk. These medications lower blood glucose and support weight loss. Records show that individuals with diabetes on these drugs have a lower dementia risk. Comparing GLP-1 drugs to metformin, studies have found that they were even more effective than metformin at reducing dementia risk.
Insulin Sprays and Dementia Treatment
Given the importance of insulin sensitivity in the brain, researchers have explored the use of insulin sprays to deliver insulin directly to the brain while minimizing effects on blood sugar. Small studies have suggested that these sprays may help improve memory or reduce brain shrinkage, but delivery methods remain a challenge, and long-term safety has not yet been proven.
SGLT2 inhibitors, a type of diabetes medication, have also been found to have superior effects on reducing dementia risk compared to GLP-1 receptor agonists. These tablets lower blood sugar by increasing sugar removal in the urine, which may help reduce inflammation in the brain.
This growing body of evidence suggests that managing diabetes may protect more than just the heart and kidneys – it may also help preserve brain function. However, questions remain about whether diabetes medications only reduce diabetes-associated dementia risk or if they could also reduce risk in individuals without diabetes.
